There are many numbers I review when evaluating the health of a dental practice, but one metric that always gives me insight into potential system gaps is the perio percentage.

Your perio percentage can tell a powerful story about your hygiene department, documentation systems, and patient care.

What Is Your Perio Percentage and Why Does It Matter?

Your perio percentage compares the amount of periodontal therapy being completed in your practice compared to preventive hygiene services.

This isn’t a number automatically calculated on a standard Dentrix report, but it is one of the numbers I always want to know during an assessment.

My friend and dental colleague Rachel Wall created a perio calculator that helps practices measure this number by reviewing codes like:

  • D1110 adult prophylaxis
  • D4910 periodontal maintenance
  • D4341 scaling and root planing
  • D4342 scaling and root planing

Once you calculate this percentage, you start getting a clearer picture of your hygiene systems.

Your perio percentage starts with having accurate data, and that begins with consistent periodontal charting. Three Reasons Your Dentrix Perio Numbers May Be Low

When I find low perio numbers during a practice assessment, it usually comes down to one of three things.

  1. Your Team Is Not Diagnosing Periodontal Disease

The first possibility is a lack of diagnosis.

This often happens because the team is missing the right diagnostic information.

For example, I worked with a practice where the perio percentage was under 5%. When I looked inside Dentrix, I could not find perio charting documentation anywhere.

No perio charts.
No measurements.
No diagnostics.

Without periodontal charting, how can you properly diagnose periodontal disease?

The answer is you can’t.

Sometimes this happens because hygienists are running behind and feel like they do not have time to complete perio charting.

When that happens, we need to evaluate:

  • Hygiene appointment flow
  • Time management
  • Clinical systems
  • Automation opportunities
  • Dentrix workflow efficiency

The goal is to create systems that allow your team to complete proper diagnostics consistently.

One of the biggest barriers I hear from hygienists is, “We don’t have enough time.” The good news is Dentrix has tools that can help streamline this process.   1. You Are Diagnosing Perio but Referring It Out

The second possibility is that your team is diagnosing periodontal disease, but you are referring treatment to a specialist.

That is completely fine.

However, we still need a way to track that.

If your Dentrix perio production looks low because treatment is being referred out, then I recommend reviewing your referral reports.

You should be able to identify:

  • How many patients were diagnosed
  • How many were referred
  • Where those patients were sent

The data should tell the full story.

  1. Your Team Has a Coding Problem

The third reason your perio percentage may be low is incorrect coding.

This is where things become more serious.

If your hygienist is performing periodontal therapy, documenting periodontal therapy, but billing the appointment as a regular prophy, that creates a major compliance concern.

Your clinical documentation, procedure completed, and billing code must match.

This is also why your Dentrix clinical notes are so important. Your documentation should always tell the complete story of what was diagnosed, what was recommended, and what treatment was completed. Now, patients always have the right to decline recommended treatment. A patient diagnosed with periodontal disease may decide not to move forward with scaling and root planing.

  • The diagnosis
  • The recommended treatment
  • The patient’s decision
  • What service was actually provided

Clear documentation protects your practice and your patients.

Use Dentrix Data to Find Hygiene System Gaps

Your perio percentage is more than just a number.

It gives you insight into your systems.

A low percentage may reveal:

  • Missing perio charting
  • Lack of diagnosis
  • Scheduling challenges
  • Coding inconsistencies
  • Documentation issues

Once you identify the gap, you can create a better workflow.
